- The program is funded by New York State Medicaid under the 1115 Waiver and delivered through the Social Care Network. We are an official partner — our role is to make sure Staten Island families receive safer, healthier home environments at no cost.

- Who qualifies? You may qualify if:
- Live in Staten Island
- Have active Medicaid
- Are part of a priority group: seniors, pregnant women, postpartum women (within 12 months), people with asthma, people with immune or chronic health conditions.
- What services are included for eligible Medicaid recipients?
- Asthma & Air Quality Support: HEPA air purifiers, allergen-reducing cleaning supplies, hypoallergenic bedding, small-area mold remediation (under 10 sq. ft.)
- Home Safety Improvements: grab bars, ramps, non-slip floor covers
- Air Sealing & Comfort: reducing drafts, moisture, and pollutants indoors
- How do I get started?
- Fill out our short intake form (we only ask if you have Medicaid, your phone/email, and what time is best to call you).
- Our team contacts you within a few days.
- If you qualify, we schedule a home visit to provide services.
- I work with patients. Can I refer someone?
Yes. If you are a navigator, hospital, or community health partner, you can refer patients directly to us. We provide:- A fast intake process
- Quick confirmation of services
- Updates on completion so you know your patients are cared for
- What if I don’t have Medicaid?
- This program is only available to Medicaid recipients under the 1115 Waiver. If you do not have Medicaid, we may be able to connect you with other community programs that can help.
- Will this affect my Medicaid benefits?
- No. These services are an add-on benefit through the 1115 Waiver. They do not reduce or replace any of your existing Medicaid coverage.
